Bipolar disorder that is hard to stabilise

When mood shifts without warning, family life reorganises itself around it: debts run up during a high, unpredictable days at home, years of treatment without a real response. Cases that have already been through several attempts call first for a careful diagnostic review.

Treatment-resistant depression

Not every depression that fails to improve is depression alone. When several treatments have been tried without result, the answer is not to repeat the same logic: it is to review the diagnosis, what runs alongside it and what sustains it. The exhaustion of those who accompany the patient counts too.

Life crises: separation, grief and serious illness

A marital or contested separation, the death of someone close, a severe diagnosis in the family. Suffering with a cause is expected — but when it stretches on for months and begins to affect sleep, work and relationships, it stops being a reaction and becomes a clinical problem. Not every case calls for medication.

Combined treatment

Medication and psychotherapy achieve more when they are not split between two consulting rooms that never speak to each other. I am a psychiatrist and an analytically trained psychotherapist, and I conduct both when the case calls for it — treating what sustains the symptom rather than only silencing it.

Mental health of those who care for others

Responsibility for another person’s life, long hours from training onwards, and the particular difficulty of asking for help within one’s own profession. I see physicians and other health professionals, and I take referrals from colleagues across specialties, reporting back to the referring doctor whenever the patient consents.
